There are just 3 simple steps to get started:

STEP 1: Complete the ELCAS claim form (click here to download it) quoting the following information:
Provider name:
Provider ID Code:
If you are still serving in the military...
STEP 2: Obtain a Claim Authorisation Note (CAN) from your line manager and education staff
If you have finished your service:
STEP 2: Obtain a Claim Authorisation Note (CAN) from your Single Service Representative
STEP 3: Once your claim is accepted, call us on 01926 889885 and send us your Claim Authorisation Note (CAN) to begin the enrolment process
